ABSTRACT

The condition of patients with severe traumatic brain injury (sTBI) complicated by corona virus 2019 disease (COVID-19) is complex. sTBI can significantly increase the probability of COVID-19 developing into severe or critical stage, while COVID-19 can also increase the surgical risk of sTBI and the severity of postoperative lung lesions. There are many contradictions in the treatment process, which brings difficulties to the clinical treatment of such patients. Up to now, there are few clinical studies and therapeutic norms relevant to sTBI complicated by COVID-19. In order to standardize the clinical treatment of such patients, Critical Care Medicine Branch of China International Exchange and Promotive Association for Medical and Healthcare and Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Chinese expert consensus on clinical treatment of adult patients with severe traumatic brain injury complicated by corona virus infection 2019 ( version 2023) based on the joint prevention and control mechanism scheme of the State Council and domestic and foreign literatures on sTBI and COVID-19 in the past 3 years of the international epidemic. Fifteen recommendations focused on emergency treatment, emergency surgery and comprehensive management were put forward to provide a guidance for the diagnosis and treatment of sTBI complicated by COVID-19.

ABSTRACT

Objective:To investigate the risk factors of venous thromboembolism (VTE) in patients with severe traumatic brain injury (TBI).Methods:A retrospective case-control study was conducted for clinical data of 180 severe TBI patients admitted to West China Hospital, Sichuan University from July 2019 to July 2021, including 137 males and 43 females, aged 18-93 years [(50.8±18.2)years]. Glasgow Coma Scale (GCS) was 3-8 points [4 (3, 6)points].VTE was presented in 95 patients (thrombosis group), but was not seen in 85 patients (non-thrombosis group). Univariate analysis was used to detect the correlation of the following data with the occurence of VTE, inlcuding sex, age, body mass index (BMI), hypertension, diabetes, laboratory indicators measured on admission and at days 3-5 after admission [hemoglobin (Hb), platelet counts (PLT), antithrombin III, prothrombin time (PT), thrombin time (TT), activated partial thromboplastin time (APTT), fibrinogen (FIB), D-dimer (D-D)], tracheal intubation or not, operation or not, volume of intraoperative blood transfusion, operation time, anesthesia time, admission to ICU or not, mechanical ventilation or not, pharmacological prophylaxis or not, deep venous catheterization or not, parenteral nutrition or not, tracheal intubation time, tracheotomy or not, pneumonia, stress ulcer, blood transfusion or not and hospital stays. Multivariate Logistic regression analysis was used to identify the independent risk factors for VTE.Results:Univariate analysis showed that the occurence of VTE was correlated with sex, laboratory indicators measured on admission (TT, FIB, D-D), tracheal intubation, operation, intraoperative blood transfusion, operation time, anesthesia time, admission to ICU, mechanical ventilation, laboratory indicators measured at days 3-5 after admission (PLT, TT, FIB, D-D), deep venous catheterization, parenteral nutrition, tracheal intubation, tracheotomy, pneumonia, blood transfusion and hospital stays (all P<0.05 or 0.01), not with age, BMI, hypertension, diabetes, laboratory indicators measured at admission (Hb, PLT, antithrombin III, PT, APTT), laboratory indicators measured at days 3-5 after admission (Hb, antithrombin III, PT, APTT), pharmacological prophylaxis and stress ulcer (all P>0.05). Multivariate Logistic regression analysis showed that higher FIB on admission ( OR=1.27, 95% CI 1.04-1.56, P<0.05), tracheal intubation ≥7 days ( OR=2.98, 95% CI 1.40-6.33, P<0.01), tracheotomy ( OR=2.49, 95% CI 1.11-5.60, P<0.05), blood transfusion ( OR=2.75, 95% CI 1.25-6.06, P<0.05) and hospital stays >14 days ( OR=3.05, 95% CI 1.36-6.85, P<0.01) were significantly related to the occurence of VTE. Conclusion:Higher FIB on admission, tracheal intubation ≥7 days, tracheotomy, blood transfusion and hospital stays >14 days are independent risk factors for the occurence of VTE in severe TBI patients.

ABSTRACT

Objective To analyze the clinical features of patients with traumatic brain injury in the Ya' an earthquake and discuss the treatment experiences.Methods Medical records of 69 patients admitted from April 2013 to May 2013 because of traumatic brain injury in the Ya' an earthquake were collected.Retrospective review was performed for age,gender,causes of injury,time from injury to hospitalization,types of injury,associated injury,treatment methods and outcomes.Results There were 47 males and 22 females.Forty-two patients (61%) were injured from falling objects.Fifty-eight patients (84%) were sent to the West China Hospital within 72 hours postinjury.Twenty-two cases (32%) sustained associated injuries.Twenty-nine patients (42%) were critically injured.Twenty-four patients underwent operation at the local hospital and twelve patients had operation at our hospital.Outcome measure using GOS one month after treatment showed 55 favorable recovery,5 moderate disability,4 severe disability,and 5 coma.Conclusions Main cause of injury is hit by falling objects during the Ya' an earthquake.Majority of the patients obtained effective treatment in the time window.GCS in combination with patients' general condition used in casualty triage and critical patients charged by neurosurgeons and treated with the cooperation of multiple disciplinary teams are helpful to successful treatment.

ABSTRACT

Objective To explore the clinical effect of percutaneous osteoplasty with SKy bone expander system in the treatment of calcaneal fracture.Methods 63 feet of 55 patients with calcaneal fractures were involved in this study.According to Sanders fracture classification including 38 feet of Sanders Ⅱ,18 feet of Sanders Ⅲ,7 feet of Sanders Ⅳ.There were 6 to 16 days interval between the injuries and the surgeries.The patients were treated by percutaneous osteoplasty with the SKy bone expander system.The standard of operation was the satisfaction of reduction and Bohler's and Gissane's angles under X-ray.Results All of 63 feet of 55 patients were followed up for average 22 months.According to the criterion of therapeutic effect,the results were as follows:excellent in 30 cases,good in 26 cases,fair in 7cases,and no poor case.The excellent and good rate was 88.9%.Conclusion Percutaneous osteoplasty with SKy bone expander system in the treatment of calcaneal fracture,especially in reduction and fixation of Sanders type Ⅱ and Sanders type Ⅲ,can recover Bohler's and Gissane's angles,significantly shorten the duration of illness,and has fast recovery and can possess satisfactory curative effect,and it is worth popularizing.

ABSTRACT

Objective To evaluate the curative effect and complications of percutaneous puncture kyphoplasty in the treatment of vertebral compression fracture in the elderly. Methods 19 cases of vertebral compression fracture in the elderly patients with disc 25 PKP technology applications were diagnosed with the accurate positioning,local anesthesia a small incision of about 0. 3cm later puncture path through vertebral pedicle to the wound vertebra,After perspective is the accurate lateral, putting into high-pressure balloon, injecting the contrast agent into the balloon with high-pressure syringe gradually. The wound vertebra is blackmai until a high degree of vertebral distraction to the recovery basis under the C-arm, and then taking the balloon out after pumping the contrast agent out. Bone cement is mixed well and injected into the cavity of the wound vertebra slowly which is the result of balloon dilation,during that time under the control of bone cement inside the wound vertebra,then puting the working column out and surgery completed. Results The low back pain of 19 patients immediately disappeared or significantly reduced after the operation, and 8 postoperative patients is up that day, 11 cases is up the following day. The degree of the wound vertebra is recovered by X-ray examination, and the bone cement evenly distributed within the wound vertebra, no spills. This group of patients are followed up for an average of 18 months(1 ~24 months) ,using visual analog and digital method to determine changes in Iow back pain. The data are paired t test,P < 0.01. It have significant difference of views.Conclusion Percutaneous kyphoplasty accurate cervical disc herniation and effective disarmament of the symptoms and signs, with trauma and rapid recovery, fewer complications, shorter course of treatment and so on, and it is a safe and effective way in the treatment of vertebral compression fracture in the elderly.

ABSTRACT

Objective To investigate the changes of serum pituitary hormones and thyroid hormones in patients with severe brain injuries during hypothermia therapy. Methods Ninety-eight patients with severe brain injuries were randomly divided into mild hypothermia group (50 cases) and control group (routine therapy group,48 cases). The levels of serum PRL,TSH,FT 3 and FT 4 of patients in the both groups were detected by two-side sandwich immunological autochemoluminescence detection system before and after treatment. The therapeutic effects were observed and compared. Results After treatment,the levels of serum PRL,TSH and FT 4 of patients in hypothermia group were (0.25? 0.02) nmol/L ,(0.7?0.2) mU/L and (15.0?5.3) pmol/L,respectively,which were significantly lower than that in control group,ie,(0.46?0.04) nmol/L,(1.1?0.3) mU/L and (21.2? 6.4) pmol/L ,respectively. The good recovery rate (62%) in hypothermia group was higher than that of control group (35%) but the death rate (22%) in hypothermia group was lower than that of control group (42%). Conclusions Mild hypothermia can inhibit the rise of serum pituitary hormones and thyroid hormones after severe brain injury and improve the therapeutic effects.

ABSTRACT

<p><b>OBJECTIVE</b>Even though phenylacetate (PA) bas been shown to inhibit the growth and induce differentiation in rat C6 glioma cell line, its mechanisms are still poorly understood. This study is aimed to identify which Hox gene is related to glioma and to observe the change in expression on mRNA level as treated by phenylasetate.</p><p><b>METHODS</b>Twenty-two kinds of Hox gene were divided into 3 groups according to their primer sequence. Semiquantitative reverse transcription- polymerase chain reaction (RT-PCR) was used to investigate the mRNA expression of Hox gene groups and some Hox gene in rat C6 glioma cell line following differentiation induced by PA. The level of Hox gene expression was expressed as ratio expression rate (RER) of Hox gene/beta-actin according to computer image analysis and the difference between C6 cells and PA treated C6 cells was analyzed by student t-test.</p><p><b>RESULTS</b>It was found that Hox genes matching to primers P2 were mildly expressed in C6 cells and the expression of HoxB2 mRNA was significantly up-regulated in PA treated C6 cells (P < 0.001).</p><p><b>CONCLUSION</b>The weak expression of HoxB2 may be involved in glioma origin and the mechanisms of PA action are correlated with transcription process in the glioma cells.</p>
